This can mean that the best diagnostic value tests can be prioritised. Please ask the clients to fill in the document, or fill it in with them, so that it is ready for the VetArtis booked work up. It is a word document so it can be filled in digitally or emailed to the client for them to do this, or you can write on a physical print out.
